To answer your question:
This question has several answers depending on which dimensioning tool you are using. To keep it simple, if you are using the "Dimension Element" tool for a linear dimension, probably the best solution is to use accudraw and just key-in the extension line length from the point where you selected the element to dimension. There is no dimension setting for establishing the length of the extension line. Essentially you can only set its offset from the dimensioned element and the extension of the extension line beyond the dimension line. The actual length is determined by the user at the time of placement.
If you use the "Linear Dimensioning" tool, there is a tool setting for the dimension offset. This defines the length of the visible extension line and does not include the distance of offset from the point dimensioned. Using this tool setting will not save you mouse clicks but it takes less mouse precision to get an exact extension line length. If you are going to use stacked dimensions you can set the "Stack Offset" to control the distance between dimemnsion lines after the base dimension is placed.
